How to refer
- Fax the referral to (510) 297-0558, or call the office at (510) 297-0550.
- Include the patient's demographics and phone number, insurance information (HMO patients need an authorization), the reason for referral and your notes. Nerve conduction studies (EMG/NCS) and imaging are helpful if already done, but not required to be seen.
- We call the patient to schedule. Patients may also call directly; a referral is not required except by HMO plans.

Who to refer for Thread Carpal Tunnel Release
Patients with carpal tunnel syndrome on exam, with or without nerve conduction studies, who want a release without an incision. Dr. Dashe practices conservative-first care, so splinting and injection remain on the table, and not every referral leads to a procedure.
He performs both thread and open release, and recommends open release for revision cases, severe long-standing compression with thenar wasting, or anatomy that cannot be safely visualized on ultrasound.
Who to refer for Thread Trigger Finger Release
Fingers that catch or lock at the A1 pulley. Thread Trigger Finger Release divides the A1 pulley through two needle punctures under ultrasound guidance: no palm incision, no stitches, in the office under local anesthetic. It is offered on the fingers, not the thumb. Injection remains an option for patients who prefer to start there.
